Please ensure that the following forms are completed in full and are forwarded with the correct documentation and relevant fee.

Licensing Act Fees

Personal Licence Application (Fee £37)
    > application for a personal licence
    > application to renew a personal licence
    > disclosure of convictions declaration

Disclosure Application Scotland forms are available from the Environment Services Dept. at Maldon District Council offices. Guidance available here.

    > application for a premises licence
    >consent of individual to be specified as a premises supervisor

                                   -  Vary a Premises Licence/Club Premises Certificate 

    > application to vary a premises licence
    > application to vary a club premises certificate

                                    -  Vary a Premises Licence to Specify a DPS (Fee £23)
    > application to vary a premises licence to specify an individual as a designated premises   supervisor
    >consent of individual to be specified as a premises supervisor

                                    - Transfer a Premises Licence (Fee £23)
    >application to transfer premises licence to be granted under Licensing Act 2003
    > consent of premises licence holder to transfer

                                     - Review a Premises Licence/Club Premises Certificate
    > application for the review of a premises licence or club premises licence

Club Premises Certificate 

    > application for a club premises certificate
    > declaration for a club premises certificate to be granted

Temporary Event Notice (TEN) Fee £21
    > Temporary Event Notice application

Other Application Forms and Notifications
    > application for a provisional statement to be granted
    > interim authority notice under the licensing act 2003
    > notification of an interest in premises under section 178

    > notification of change in name and address
    > notification of change in name and address - premises licence
    > notification of change in the name of club and or alteration made to the club rules
